Helping people to remain in their homes and communities for as long as possible is the overarching goal of aging programs in our state. Services for those needing assistance in their homes vary from helping with housekeeping to more intensive personal care services such as help with bathing, dressing, and eating. Medical and financial eligibility for each program varies.
Lighthouse is designed to assist those seniors who have functional needs in their homes, but whose income or assets disqualify them for Medicaid services. The Lighthouse Program, available in each county, is funded entirely by state monies and provides support in four areas: personal care, mobility, nutrition and housekeeping.
An individual may receive up to sixty hours of service per month, based on a client assessment and resources available. To participate in the program one must be at least sixty years of age and meet the functional eligibility need. Lighthouse has a sliding scale fee reimbursement in place. - Environmental Support (Light Housekeeping)
The Medicaid Personal Care Program includes hands-on, in-home services provided to those who are medically and financially eligible. A medical assessment is completed by a physician, assets can be no greater than $2,000 and income can be no greater than $674 per month.

The Case Management Department at Berkeley Senior Services evaluates social, environmental service and support needs of clients. He/She develops an individualized Service Coordination Plan (SCP) detailing all services to be provided.
The Case Manager will assess and identify your needs, help you plan for your care and advocates for and connects you with services or resources in the community.
He/She coordinates the delivery of care, eliminates fragmentation of services and assures appropriate use of resources.
He/She proactively identifies problems and coordinates services that provide appropriate high quality care to meet the individualized and often complex needs of the client.
The West Virginia Medicaid Aged and Disabled Waiver (ADW) program is an in-home care program that is an alternative to nursing home care. The Program provides in-home assistance with activities of daily living and other areas of personal need. Homemaker staff comes to your home to assist with your personal needs. This means that you have the similar types of needs as someone in a nursing home but it does not mean that you must go into a nursing home. You are provided with a Case Manager, Homemaker Registered Nurse and a Homemaker.
- Homemaker RN will complete assessments of the ADW member at regular intervals to ensure that the member’s plan of care is meeting his/her needs by the Homemaker.
- Homemaker will assist you with bathing, dressing, grooming, eating, fixing your meals or other personal care needs.
Eligibility requirements are as follows :
- The individual must be 18 years of age and older
- The individual must be a resident of West Virginia
- The individual must be assessed by a medical professional and that individual is determined to have five (5) or more deficits of their Activities of Daily Living on the Pre-Admission Screening Form i.e. PAS-2000
- The Individual must be assessed by an Economic Services Worker at the county Department of Health and Human Resources (DHHR) offices; individual assets cannot exceed $2,000 and gross monthly income cannot exceed $2,022 (effective January 2009) per month
